Abstract Cilj istraživanja: Procijeniti mišljenja studenata fizioterapije o temporomandibularnim
poremećajima.
Nacrt studije: Presječno istraživanje.
Ispitanici i metode: Studenti fizioterapije Fakulteta za dentalnu medicinu i zdravstvo
Sveučilišta Josipa Jurja Strossmayera u Osijeku.
Rezultati: Prisutnost okluzije kod TMP-a pokazala se kao značajan čimbenik povezan s
parafunkcijskim kretnjama, pomakom zglobnog diska, osteoartritisom, fizioterapijom te
problemima s cervikalnom kralježnicom. Taj nalaz potvrđuje okluziju kao ključni element u
etiologiji TMP-a, što implicira potrebu za pažljivim razmatranjem tog čimbenika u
dijagnostici i liječenju. Studenti fizioterapije naglasili su potrebu za upotrebom nesteroidnih
protuupalnih lijekova (NSAID) u kontekstu stresa i muskuloskeletnih problema, čime su
istaknuli važnost tih lijekova u ublažavanju simptoma povezanih s TMP-om. Ta povezanost
ukazuje na kompleksnu interakciju između psiholoških i fizičkih čimbenika u upravljanju
TMP-om te naglašava potrebu za holističkim pristupom liječenju. Stres se pokazao značajno
povezanim s pojavom krepitacija ili škljocanja u zglobu, što sugerira njegovu ulogu u razvoju
simptoma TMP-a. Ta povezanost dodatno potvrđuje kako psihološki stres može biti okidač ili
pogoršavajući čimbenik za fizičke manifestacije poremećaja. Pomak zglobnog diska i osteoa
rtritis varijable su koje su značajno povezane s dobi, što implicira da se TMP češće javlja kod
starijih osoba. Taj je nalaz važan za razumijevanje kako se prevalencija i priroda TMP-a mogu
mijenjati s godinama te ukazuje na potrebu za prilagodbom terapijskih pristupa starijim
pacijentima. Funkcionalnost zgloba pokazala se kao kritičan čimbenik, s nepomičnim dijelom
zgloba koji pokazuje značajnu povezanost s TMP-om. To naglašava važnost održavanja i
poboljšanja funkcionalnosti zgloba kroz različite terapijske intervencije kako bi se smanjila
učestalost i težina simptoma. Traumatski poremećaji zgloba pokazali su značajnu povezanost
s uznapredovalim stadijem TMP-a, što ukazuje na složenost liječenja tih poremećaja. Taj
nalaz sugerira da traumatski TMP zahtijeva specifične, često složenije terapijske pristupe koji
se razlikuju od onih za manje ozbiljne oblike poremećaja. Uznapredovali stadiji TMP-a
također zahtijevaju složenije pristupe liječenju, što je dodatno potvrđeno negativnom
korelacijom s učinkovitosti fizioterapije u tim stadijima. To implicira potrebu za razvojem i
primjenom naprednih terapijskih metoda i tehnologija u liječenju pacijenata s teškim oblicima
TMP-a. Fizioterapija se pokazala kao ključna uloga u liječenju muskuloskeletnih problema,
uključujući TMP. Njezina važnost leži u sposobnosti da smanji bol, poboljša funkcionalnost
zgloba i poveća opću kvalitetu života pacijenata. Rezultati naglašavaju potrebu za
integriranim pristupom u kojem fizioterapija zauzima središnje mjesto u sveobuhvatnom
planu liječenja TMP-a.
Zaključak: Rezultati istraživanja pružaju važne smjernice za daljnja istraživanja i kliničku
praksu, usmjeravajući stručnjake prema holističkom i individualiziranom pristupu u
dijagnostici i terapiji temporomandibularnih poremećaja.
Abstract (english) Objectives: To evaluate the opinions of physiotherapy students about temporomandibular
disorders.
Study design: cross-sectional survey
Subjects and methods: Physiotherapy students of the Faculty of Dental Medicine and Health
at Josip Juraj Strossmayer University in Osijek.
Results: The presence of occlusion in TMD was a significant factor associated with
parafunctional movements, displacement of the articular disc, osteoarthritis, physiotherapy,
and problems with the cervical spine. This finding confirms occlusion as a crucial element in
the etiology of TMD, which implies the need for careful consideration of this factor in
diagnosis and treatment. Physiotherapy students emphasized the need for the use of
non-steroidal anti-inflammatory drugs (NSAIDs) in the context of stress and musculoskeletal
problems, highlighting the importance of these drugs in alleviating symptoms associated with
TMD. This association indicates a complex interaction between psychological and physical
factors in managing TMD and emphasizes the need for a holistic approach to the treatment.
Stress is significantly associated with crepitations or clicking in the joint, suggesting its role
in the development of TMD symptoms. This association further confirms that psychological
stress can be a trigger or aggravating factor for the physical manifestations of the disorder.
Disc displacement and osteoarthritis are significantly associated with age, implying that TMD
occurs more often in elderly patients. This finding is significant for understanding how the
prevalence and nature of TMP may change with age and indicates the need to adapt
therapeutic approaches to older patients. Joint functionality was a critical factor, with the
immobile part of the joint showing a significant association with TMD. It highlights the
importance of maintaining and improving joint functionality through various therapeutic
interventions to reduce the frequency and severity of symptoms. Traumatic joint disorders
showed a significant association with the advanced stage of TMD, which indicates the
complexity of the treatment of these forms of disorders. This finding suggests that traumatic
TMD requires specific, often more complex therapeutic approaches that differ from those for
less severe forms of the disorder. Advanced stages of TMD also require more complicated
treatment approaches, which is confirmed by the negative correlation with the effectiveness of
physiotherapy in these stages. It implies the need for more advanced therapeutic methods and
technologies in treating patients with severe forms of TMD. Physiotherapy plays a significant
role in treating musculoskeletal problems, including TMD. Its importance lies in its ability to
reduce pain, improve joint functionality, and increase general quality of life. These results
emphasize the need for an integrated approach in which physiotherapy is central to a
comprehensive TMD treatment plan.
Conclusion: The research results provide important guidelines for further research and
clinical practice, directing experts towards a holistic and individualized approach in the
diagnosis and therapy of temporomandibular disorders.
