Abstract	CILJ ISTRAŽIVANJA: Cilj ovog diplomskog rada je bio ispitati korelaciju između virtualne biopsije uprabom konfokalne laserske endomikroskopije (pCLE) i „slijepe“ biopsije, tj. patohistologije kod pacijenata oboljelih od Barrettovog jednjaka. USTROJ STUDIJE: Istraživanje je retrospektivno opservacijsko. ISPITANICI I METODE: Ispitanici su pacijenti s refluksnim smetnjama i dokazanim postojanjem Barrettovog jednjaka kojima je u razdoblju od 2018. do 2020. godine učinjena konfokalna laserska endomikroskopija. REZULTATI: Prosječna dob oboljelih od Barrettovog jednjaka prema muškom i ženskom spolu je od 51 do 65 godina, tj. 47,1 %. Vidljivo je kako značajno više ispitanika ima GERB kao faktor rizika za nastajanje BE-a (76,5 %). Iz rezultata je vidljivo da najveći udio ispitanika ima pravilne vrčaste stanice prilikom upotrebe konfokalne laserske endomikroskopije (pCLE), njih 82,4 %. Analizom patohistološkog nalaza kod najvećeg broja ispitanika bila je prisutna intestinalna metaplazija bez displazije (IM, 47,1 %) i displazija niskog stupnja (LGD, 41,2 %). Prema dobivenim rezultatima najveći broj ispitanika, njih 47,1 %, podvrgnut je endoskopskoj terapiji RFA. Preporučena terapija inhibitorima protonske pumpe (IPP-om) najviše je bilo dvostrukom dozom za njih (76,5 %). Unutar konkretnog rada vidljivo je kako nema značajnih razlika u distribuciji nalaza virtualne biopsije i patohistologije (P=0,111). ZAKLJUČAK: Možemo zaključiti upotrebom virtualne biopsije pCLE da prisutnost vrčastih stanica crijevnog tipa utvrđuje dijagnozu Barrettovog jednjaka. Identificira mjesta intestinalne metaplazije i displazije. Upotreba pCLE može pomoći u određivanju histologije u stvarnome vremenu te time gastroenterolozi mogu odmah pristupiti adekvatnome terapijskom postupku bez čekanja patohistološkog nalaza.
Abstract (english)	OBJECTIVE : The aim of this thesis was to examine the correlation between virtual biopsy using confocal laser endomicroscopy (pCLE) and ‘blind’ biopsy, i.e. pathohistology in patients with Barrett's esophagus. TYPE OF STUDY: The research is retrospectively observational. RESPONDENTS AND METHODS: The subjects are patients with reflux disorders and a proven existence of Barrett's esophagus who had gone through confocal laser endomicroscopy in the period between 2018 and 2020. RESULTS: The average age of patients with Barrett's esophagus by sex is from 51 to 65 years, ie (47,1 %). It can be seen that significantly more subjects have GERD as a risk factor for developing BE (76,5 %). The results show that the largest proportion of subjects have regular goblet cells when using confocal laser endomicroscopy (pCLE), 82,4 % of them. Analysis of pathohistological findings in the largest number of subjects included showed intestinal metaplasia without dysplasia (IM, 47,1 %) and low-grade dysplasia (LGD, 41,2 %). According to the obtained results, the largest number of subjects (47,1 %) underwent endoscopic RFA therapy. The recommended proton pump inhibitors therapy (PPIs) was a maximum of double dose for them (76,5 %). Within the concrete work, it is evident that there are no significant differences in the distribution of virtual biopsy and pathohistology findings (P = 0,111). CONCLUSION: We can conclude by using virtual pCLE biopsy that the presence of intestinaltype goblet cells determines the diagnosis of Barrett’s esophagus. It identifies sites of intestinal metaplasia and dysplasia. The use of pCLE can help determine the histology in real time and thus gastroenterologists can immediately access an adequate therapeutic procedure without waiting for a pathohistological finding.
