Objective: To present the process of health care of patients with colostomy and education after discharge, as well as to present the established nursing diagnosis in the process of health care. Planned and implemented interventions in preoperative and postoperative health care are aimed at solving the patient’s identified problems. Case description: During hospitalization, the following diagnoses were made in the health care process: fear about the outcome of the surgical procedure, pain of the surgical incision, high risk of infection associated with surgical incision and drain placement, reduced ability to perform personal hygiene in / with reduced mobility due to the secondary surgery, reduced ability to dress due to reduced mobility following secondary surgery, non-acceptance of one's own appearance in connection with altered physical appearance following secondary colostomy, and ignorance of colostomy care due to the lack of specific knowledge. The patient expressed a reduced level of fear during hospitalization after the performed interventions, as well as reduced levels of pain. What is more, there was no infection, and on the last day of hospitalization, he performed personal hygiene and dressed himself. The patient also expressed greater self-esteem. Following a lesson that was given to him by the community nurse, the patient acquired the necessary specific knowledge about colostomy care. Conclusion: In order to ensure a high-quality perioperative care and a high-quality life of patient with colostomy, it is extremely important to ensure a holistic, individual approach to the patient, and at the same time provide physical, psychological and social assistance with ongoing education.
